In my partner's case he has offending on and off for over ten years, claims it was progression from porn and getting thrills from taboo topics. My partner was only flagged to the police after someone reported a photo in a chatroom, it took the police a year to arrest him.

I'm not sure the time frame for others but at the end of the day those who offend and know what they are doing is wrong will hide it as best as they can. And it is so hard to regulate and police. The arrest rates are in their hundreds a month.

Ofc I wish my partner never looked at and for iioc, but secondly I wish he was either caught or given help so much sooner. He ended up having hundreds of images in each category, it became a stupid coping mechanism for him to go into fantasy land. He would catfish and chat to other pretending to be young, share images and videos as a form of currency to get more extreme footage etc. He didn't just view iioc. It was all manners of perverse pornography- including animals.

I do have that paranoia that he looks at young people and finds them attractive. I am so uncomfortable being at the beach or near kids are swimming. I have told him it makes me uncomfortable and we avoid these situations where possible. He tells me has no interest in children but I will always have that doubt. I just have to have faith he will not reoffend. He has had the help to find alternatives to his need for escapism.
