That's so tricky with working for the same company and your ex not being open. I'd personally say be as vague as possible unless you want your manager to know the details. I wouldn't mention police and social services unless you're planning to say it's due to your ex as it could be detrimental to you as you're hoping your manager will presume it's down to your ex but there is a chance they will presume it could be down to you. Your manager shouldn't really ask for additional details. Saying your struggling with your mental health and will need support in the future when having time off for appointments should be enough... Maybe via email to take away the chance of your manager pressuring you for details you don't want to or have to give.
