Does ss case re open when released from prison

My daughters case with ss was closed a couple of days after he was sent to prison but there was still the restriction of supervised contact in place. His mum agreed to supervise but has been in hospital for weeks and can't see this being viable now. I would really like to work towards him having unsupervised contact but don't know if this is going to be possible. Iv heard alot of people say they were re referred to ss on release. Is this a standard procedure for all serving a sentence?
SW told us that cases don't just get reopened after sentencing/ prison etc unless another referral comes in and then another assessment is carried out.

I've been told his probation officer will refer bk to SS however my SW wrote in her report she recommends the same access as he had prior to his sentancing (unlimited supervised contact) and just for another assessment to be completes if/when we want unsupervised or of he moves home. So should just be a paperwork thing to let them know when he's released. Fingers crossed anyway.
