OASYS and assessments

Sorry, I've made another post. I'm looking to find out what probation do to assess, monitor and help offenders and can anyone tell me about the Oasys assessment please? Is it carried out at the start of the sentence and is it a continuous assessment until probation ends? Can we find out the risk level/score they have given and does this change through time with probation and VISOR?
We have a letter saying they think a disclosure should be made. This is the first we have heard of this. Could it be that something has been raised from assessments? but my person says they just ask how he's been and what he's been doing since last appointment. The appointments had lessened too and are now half way through their probation. Everyone knew about the activity group at the beginning and we were told person didn't needed to disclose. I thought we were through worst of things.

They have to do the assessment within a few weeks of meeting the person and it normally gets updated once a year. But they won't necessarily tell the person they are assessing them. They ask seemingly innocent questions about the person's life, but all the time they are risk assessing that person.
They will tell him the risk level if he asks and it does change. Most people start as medium risk, then get reduced to low risk eventually, if they co operate with Probation.
As for the disclosure, this can sometimes be triggered by somone new getting involved with the process. Perhaps a senior probation officer has reviewed the case and flagged the activity group.
Everytime a new PO or a new VISOR comes in, they bring their own opinions, so what was previously seen as not being a problem, now becomes a problem. The person may not have changed at all, but the people supervising them have and that always seems to cause issues.

Thanks edel2020. The VISOR has changed I believe and regarding probation appointments they seemed to be less frequent.
My son appears to have zero paperwork from probation. So I don't know if they share their opinion with him. And they say he doesn't engage,despite attending every appointment & course/agency suggested.
